Black hairy tongue is a benign, temporary condition caused by failure of normal shedding of the filiform papillae on the tongue, leading to their elongation and accumulation of bacteria, fungi, food debris, or stains from smoking, coffee, tea, alcohol, or certain medications (especially antibiotics). It is commonly associated with poor oral hygiene, dry mouth, tobacco use, and prolonged use of oxidizing mouthwashes. The condition is usually painless and resolves with improved oral hygiene, gentle tongue brushing or scraping, adequate hydration, and avoidance of triggering factors; medical evaluation is needed only if it persists or is associated with pain or ulcers.
